When to Keep Your Child Home From School

Here is the Parent Fact Sheet for Medication at School.  If you would like to send either prescription or nonprescription medication into school to be administered to your child you will need to complete and return the form listed below.  Please note there are specific forms for Epi-Pens and Inhalers/Nebulizers.
