Required vaccines are available for your children at the Prince George’s County Health Department.

ELIGIBILITY

• Prince George’s County provides immunizations to residents:
• Covered by or enrolled in Medicaid or medical assistance programs;
• Who are 2 months to 18 years old and are uninsured or underinsured;
• Who are Native American (American Indian) or Alaskan Native;
• Who have health insurance that does not cover (pay for) vaccines

FEES

Fees are based on a sliding scale.

Important documents to bring:

• Parent's ID
• Child’s ID (Birth Certificate or Passport)
• Child’s Entire Immunization Record
• Medical Assistance Card or Managed Care Organization (MCO) Insurance Card
• Proof of Residency
• Proof of Income (If your child does not have insurance)
