Registration

To Enroll your child, you will need:   

Pre K Program: 

• Pre K Application -Completed & Signed See or Call Ms. Watson in Main Office for Application. 
• Proof of Child's age (Birth Certificate - Child must be 4 yrs of age by September 1, 2020) 
• Proof of current income of Parents/Guardians of child
• Proof of Residency 
• Completed Health Forms
• Shot Records

Kindergarten thru 6th Grade:

Your Child CAN NOT Be Enrolled Without These Items!        

• Birth Certificate (With the Seal)
  
  • (Birth Registrations & Birth Certificate Cards are NOT acceptable)
• Social Security Card
• Valid ID (Of Registering Parent/Guardian)
• Proof of Residency
  
  • Current Lease or Deed (With Deed need a Current Utility Bill)
  • Current Rental Receipt on Company's Letter Head
  • Current Shared Housing Disclosure Letter (Only if Registering Parent Name is not on Lease)
• Shot Records (Must Be Current)
• Last Report Card
